Service Delivery Process
- Identification & Referral
Children are identified through outreach, schools, child-protection agencies, shelters, police, community organizations and other approved referral partners. - Immediate Safety Assessment
Each child receives an urgent assessment to determine immediate risks, medical needs, trafficking/exploitation concerns and appropriate accommodation. - Intake & Comprehensive Assessment
A trained social worker/case manager assesses the child’s family situation, housing, education, health, psychosocial needs and protection concerns. - Safe Placement
The child is placed in an appropriate Angel Home/Dream Home, based on age, gender, vulnerability, protection needs and available capacity. Children should be housed in small, family-like settings where possible rather than institutional environments. - Individual Care Plan
A written care plan is developed for each child, identifying education, health, psychosocial, protection and family-placement goals. - Stabilization & Support
The organization provides housing, food, clothing, education, healthcare access, counselling, mentorship and other essential services. - Family Tracing & Permanency Planning
Where appropriate and safe, the organization works with authorized child-protection professionals to trace family members and assess possibilities for reunification, kinship care, foster care or another permanent safe placement. - Ongoing Monitoring
Each child’s safety, wellbeing, education, development and placement are regularly reviewed and documented. - Transition & Follow-up
Children returning to families or transitioning to alternative care receive follow-up support. Older adolescents receive education, vocational training, life-skills and preparation for adulthood.
Safeguarding Principle
The best interests, safety, dignity and rights of the child will guide every placement and service decision. ACT Foundation will maintain strict child-protection policies, staff/volunteer screening, appropriate supervision, confidentiality, incident-reporting procedures and zero tolerance for abuse, exploitation or trafficking.
Expected Outcomes
- Vulnerable children removed from unsafe living conditions
- Reduced exposure to trafficking, exploitation and street homelessness
- Increased school enrollment and retention
- Improved physical and psychosocial wellbeing
- Increased family reunification or safe permanent placement
- Improved preparation for independent and productive adulthood
ACT Foundation’s vision for Angel Homes/Dream Homes is simple: No vulnerable child should have to sleep on the street or remain in an unsafe environment when a safe home and caring support can be provided
